How to request payment from the Devon PTO
You have two options:
- Complete this online form and email your receipts/invoices electronically
- Print out hard copy of form and submit by hand/mail

Complete online form. After the form is submitted please email your receipts/invoices to the PTO Treasurer.
Print out hard copy of form and submit by hand/mail
- Complete the “Devon PTO Treasurer Reimbursement Form.”
- Attach the original receipts to the request form.
- Return the completed form and receipts to the PTO Treasurer:

- The PTO Treasurer school mailbox is checked at least once per week. However, if I need to process the request as quickly as possible, please send an email to the PTO Treasurer.
Please allow extra processing time for checks over $500 since a second signature is required.
